Through this site we may collect information about your aesthetic concerns, treatment history, goals, and optional photographs. Combined with your contact details, this is health information and we treat it as confidential.

For treatment: so our clinical team can prepare and deliver your care.

For payment and operations: to schedule, bill, and run the practice, including quality review and staff training.

As required by law: where disclosure is required by state or federal law, or in response to a valid legal process.

With your written authorization: any other use, including marketing, testimonials, or public before-and-after images, requires your separate written authorization, which you may revoke at any time.

You have the right to see and get a copy of your records, to ask us to correct them, to ask for a list of certain disclosures, to request confidential communications, to request restrictions on certain uses, and to receive a paper copy of this notice.

You may revoke an authorization in writing at any time, except where we have already acted in reliance on it.

If you believe your privacy rights have been violated, contact us and we will investigate. You will not be penalized for filing a complaint.
